Our CQI Program Manager serves as leader, mentor and role model. Coordinates the continuous quality improvement (CQI) activities for The Company; Provides training and guidance regarding CQI; Assists in tracking and trending CQI data, including CCE and mortality review data; Creates Home Office auditing reports, which document, report and communicate compliance with the The Company CQI program requirements, The Company Policies and Procedures (both administrative and clinical), and contract requirements, as identified and requested; Collaborates with sites to develop, implement and communicate quality improvement plans to bring site policies and procedures into compliance with the quality requirements; Assists sites in developing ad hoc screens as needed; Monitors Home Office grievances; Submits required reports to both the The Company Home Office CQI Committee within appropriate timeframes.Education:

  • Graduate from an accredited School of Nursing - Bachelor's degree preferred
  • OR
  • Bachelor's or higher degree in a health-related field from an accredited program (Social Work, Psychology, Counseling, etc.)Experience:
    • Minimum of three (3) years clinic experience preferred
    • Computer literacy required
    • Familiarity with Microsoft Office essentialLicenses/Certifications:
      • Current CPR certification
      • Establishes the The Company CQI program in coordination with the Chief Medical Officer. Tracks and trends performance on the CQI measures for all sites
      • Identifies potential problems, establishes priorities, initiates effective problem solving based on clinical knowledge and informs The Company Leadership of trends
      • Develops Improvement Plans for the company when system-wide issues or trends are identified
      • Works with the The Company People Development to develop and execute plans for employee development to improve The Company performance on CQI measures
      • Performs audits when necessary for company initiatives. Drives the Peer Review process for the company.
      • Works collaboratively with the Vice President for Legal Affairs and Clinical Focus regarding the The Company Patient Safety program and Critical Clinical Events
      • Coordinates the collection of monthly quality improvement screens
      • Submits quarterly reports of audits conducted
      • Chairs the Home Office CQI committee and ensures the accuracy of CQI Committee Meeting minutes
      • Ensures that the CQI minutes reflect implementation of improvement plans
      • Reviews and trends medication errors reported as CCEs
      • Prepares and submits requested reports in a timely manner
      • Reviews established policies and procedures and provides recommendations as needed
      • Communicates information to Home Office, Regional, and Site leadership, and other staff as necessary and in a timely manner
      • Respects dignity and confidentiality of patients
      • Maintains the confidentially of CQI work product
      • Maintains a log of professional staff requiring Peer Review and support the sites by notifying when these are required
      • Trends information from accreditation audits
      • Works collaboratively with Regional and Site staff to help meet CQI contract requirements
      • Works collaboratively with clients on quality issues
      • Shares knowledge and skills with others and brings creativity and innovation to practice
      • Routinely assesses the educational needs of staff through CQI trends and aids in the development of educational objectives and strategies to meet those needs
      • Collaborates in the development, delivery, and evaluation of educational programs or tools with the The Company Nurse Educator
      • Develops and annually reviews a program plan with measurable objectives and determines appropriate resources needed to accomplish the proposed plan
      • Maintains and updates the CQI Manual for the company
      • Supports and promotes the CQI program at the sites via education to key personnel
      • Considers both clinical outcomes and financial implications when making recommendations for changes in practice
      • Makes decisions and performs activities in an ethical manner
      • Will work cooperatively with Home Office, Regional, and Site level staff to accomplish site, regional, and company goals
      • Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
      • Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.
      • Must be alert at all times; pay close attention to details.
      • Must be able to work under stress on a regular or continuous basis.
