Job Description:

In this role, you will be responsible for the oversight and collaboration with the Care Coordination team in the development, execution, and continuous review of a comprehensive care coordination model for enrollees with serious illness. This position will oversee the impact on enrollees and caregivers across the United States who are being delivered telephonic and virtual visits by qualified and trained staff. Responsibility includes, but is not limited to implementation, monitoring, and evaluating activities and performance in building standardized processes that deliver high quality care to each enrollee and their caregivers. Full-time office-based position with some flexibility to work from home.
Primary Responsibilities: --- Provides supervision and coordination in standardizing processes to drive and achieve performance targets. --- Reviews and assigns cases to ensure appropriate caseload distribution for all Care Coordinators --- Assists with the implementation of the care coordination program and monitoring for continuous program improvements. --- Works with the Director to plan, educate, implement, and provide recommendations for budgetary and departmental needs. --- Ensures enrollees and families receive quality care by reviewing documentation, consulting with staff, reviewing care coordination plans, conducting/participating in multidisciplinary team conferences, and reviewing enrollee satisfaction reports. --- Monitors operations for compliance with licensure, accreditation, and regulatory standards. --- Develops and implements care coordination action plans as needed to improve staff development, quality, and outcomes of the care coordination program. --- Hires, develops, and manages employees for safe, timely, efficient, equitable, and enrolleecentered care delivery. --- Provides back-up as needed for coverage. --- Performs other related duties and participates in special projects as assigned.
Required Qualifications: --- Licensed RN with applicable state licensure and willingness to obtain license in all applicable states and territories. --- 2-4 years' experience in care coordination/case management or in clinical or community health setting --- Experience in effective data collection, organization, and evaluation systems for monitoring quality of care. --- Proficiency in software applications that include, but are not limited to, Microsoft Word, Microsoft Excel, Microsoft PowerPoint Prospero RN Supervisor, Care Coordination JOB DESCRIPTION --- Demonstrated ability to assist with focusing activities on a strategic direction as well as to develop tactical plans. --- Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action --- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others. --- Ability to maintain confidentiality of all medical, financial, and legal information. --- Dependable and reliable work ethic
Preferred Qualifications: --- Master's degree preferred, BSN minimum --- Certified Case Manager (CCM) preferred --- 2+ years' experience in Medicaid and/or Medicare health care and insurance industry, including regulatory and compliance requirements preferred --- Bilingual in English and Spanish desired --- Knowledge of community health services
