the comfort of their homes, local offices collaborate with skilled
nursing facilities to help patients regain independence and
transition home after rehabilitation. Tennessee Quality Care
operates in 31 counties across middle and west Tennessee through
multiple branch offices. The Tennessee Quality Care division office
is based in Parsons, Tennessee.POSITION SUMMARY:The Care
Coordinator is to serve as a liaison to staff members, physicians,
and health care facilities. This position acts as an ongoing
resource for the Company.ESSENTIAL FUNCTIONS: To perform this job,
an individual must perform each essential function satisfactorily
with or without a reasonable accommodation; including but not
limited to:Assist with maintaining accurate records on all referral
sources, status of existing or pending referrals, admissions,
discharges, and hospitalized patients as neededEnsure that a
primary liaison is maintained between the Company and referral
sourcesInvolve other community agencies in the referral process as
neededCompile patient information from referral sources Including
specific patient demographics, insurance and/or payor sources in
order to ensure quality of communications, information and care for
patientsAssist with ensuring patients meet the face-to-face
encounter regulationsProvide skilled nursing care to patients in
accordance with physician's orders and in compliance with company
policies/procedures, state and federal regulations if requestedMay
participate in on-call rotationComplete clinical record
review/auditsManage and document phone calls from physicians,
clinicians, patients, referral sources and communicate patient
updates/new orders to cliniciansFollow up on labs and other
clinical diagnostic test, physician contact, and significant
changes in the patient condition to ensure adequate physician
notification, follow up and needed plan of care modifications and
communicates such to cliniciansReview clinician visit notes to
ensure timely, complete appropriate and accurate submission of
documentation by visiting staffCommunicates findings/trends to
managerReview, evaluate and supervise service delivery to ensure
appropriateness of care and utilization of services, equipment and
supplies through activities such as medical record review and case
conferencesRequired Work Experience:3 years' experience as a
licensed practical nurse or RN preferred1 year home health
experience required1 year case management
preferredLicensing/Certification/Education Requirements:Current
State of Tennessee License Practical NurseCurrent State of
Tennessee Registered Nurse License preferredCPR
CertificationCurrent state driver's licenseCurrent automobile
insurance according to company policyPhysical Requirements:Job may
require frequent lifting of objects up to 40 poundsSubject to
