Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment programs and coordinates care with all physicians and clinicians to provide a whole person approach. ResponsibilitiesPerforms ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and determine level of functioning and appropriateness of patient for home care.Develops and evaluates the plan of care in partnership with the physician, patient, representative (if any), and caregiver(s). Revises the POC based on evaluations and ongoing assessment data.Coordinates care and documents coordination with all physicians and other health care clinicians involved in the care of the patient as needed and appropriate.Provides patient caregivers and family education.Documents, prepares, and completes clinical notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.Provides treatment and services according to state and federal regulations and professional standards.Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.Provides supervision to Physical Therapy Assistants and Aides, when applicable, in the performance of patient care that is consistent with the patient's needs.Meets mandatory continuing education requirements of the Agency and licensing board.Uses effective interpersonal relations and communication skills.Promotes the Agency philosophy and administrative policies.Demonstrates commitment, professional growth, and competency.Participates in the Agency's QAPI program.Participates in Agency-sponsored in-service training. QualificationsEducation: Graduate of an accredited Physical Therapist (PT) therapy program.Licensure: Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC.CPR Certification: Requires current CPR certificate, and the ability to maintain a current CPR certificate.Experience: At least one year's experience as a Physical Therapist in an acute care, rehabilitation setting, or (preferred) home health.Skills: Therapy skills as defined as generally accepted standards of practice. Tech-savvy with the ability to learn quickly.Transportation: Reliable transportation, current Driver's License, and valid and current auto liability insurance.
