the Primary Care delivery program that seeks to implement a new
model of patient and family centered health care delivery for
primary care that is team-based, comprehensive, and designed to
fully meet the complete primary care health and wellness needs of
patients - - Provide consultative behavioral health care for all
enrollees in the Patient Centered Medical Home Port (MHP) - -
Identify, treat, manage, and follow up with behavioral or
psychosocial difficulties experienced by patients in the - -
Medical Home Port Clinic. - - Provide timely and succinct feedback
to primary care physicians and/or primary care managers (PCMs)
regarding - - consultations findings and recommendations. - - Apply
knowledge and experience in the relationship between medical and
psychological aspects of health and - - disease while engaging in
behavioral health care consultation with enrollees and PCMs. - -
Apply knowledge in diagnostic issues related to psychological
disorders resulting from, or intensified by, injury or - - disease
while engaging in behavioral health consultation with enrollees and
PCMs. - - Apply the biopsychosocial model of assessment to the
primary care setting. - - Formulate diagnostic and treatment
recommendations and presents findings to treatment teams (i.e.,
physicians, - - social workers, psychologists, Corpsmen, and
nursing staff, as appropriate). - - Apply knowledge of
psychological assessment, psycho-diagnostic evaluations, and
psychotherapy. Demonstrate - - specialized knowledge of
evidence-based treatment for general behavioral health problems
(e.g., depression and - - anxiety) within the primary care setting.
Apply knowledge of evidence-based treatment for areas of behavioral

Provide concise documentation of care and recommendations in
patient's medical record - - Provide appropriate behavioral health

Determine the appropriateness of the patient receiving treatment in
the primary care setting and assists with - - implementation of
treatment plans. - - Formulate behavioral health interventions
appropriate to primary care setting and assists with patient
implementation - - of prescribed treatment plans. - - Provide brief
follow up consultation following treatment conclusion, including
relapse-prevention education. - - Provide and teach curriculum
content for classes that promote education and skill building to
enhance enrollee - - Proactively collaborate and integrate with
members of their MHP team, other departments, and other

model. - - Maintain effective communication with staff, patients,

Doctor of Psychology (PsyD). - - Graduate from an American
Psychology Association accredited college or university with a
doctoral degree in clinical or counseling psychology. - - ---
Internship/Residency: Successful completion from an internship in
psychology accredited by the American Psychological Association. -
- Certification: BLS - American Heart Association Basic Life
Support (BLS) for Healthcare Providers - - one-year experience as a
clinical psychologist - - Licensure/Registration: Have and maintain
