_*LPN - $5,000 Sign On Bonus*_*Available Shift: 3 PM to 11 PM*_Cordova Wellness and Rehabilitation Center is a leading senior care provider dedicated to providing a compassionate, patient-centered health care experience through a culture of caring, quality, and excellence in the communities we serve._*LPN Position Summary*A Licensed Practical Nurse is responsible for providing basic medical attention to patients and assessing their wellbeing. Their duties include checking vital signs, recording a patients medical history, and often includes the following responsibilities: Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel. Participate in admissions, discharges and transfers of residents as required. Assess residents conditions, prepare, and administer medications per physician orders. Initiate, review, and update care plans. Report any changes in a residents condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned. Perform other assigned duties given by clinical or administrative leadership.
