Job Description:

underserved communities. As a Primary Care Provider, either as a
Nurse Practitioner or Physician Assistant, you will play a crucial
role in our community-oriented medical services team. This position
offers a unique opportunity to make an impact on patient lives,
providing both urgent and primary care under our holistic health
model. We are committed to enhancing patient outcomes and ensuring
that our services are accessible to all. Conduct comprehensive
patient assessments, diagnose, and treat a variety of illnesses and
injuries. Develop and implement patient management plans, record
progress notes, and assist in provision of continuity of care.
Prescribe medications following the organization's protocols and
state’s regulatory bodies. Advise patients on health maintenance
and disease prevention, nutrition, and lifestyle choices.
Collaborate with other healthcare professionals to prepare for and
respond to emergency cases. Excellent patient management skills
Proficiency in electronic health record (EHR) systems Strong
decision-making abilities and diagnostic skills Ability to work
independently and as part of a collaborative team Effective
communication skills, both verbal and written Medical Insurance
Dental Insurance Vision Insurance 401k Valid Nurse Practitioner or
Physician Assistant state license. Board Certification (AANP or
NCCPA) CPR and Basic Life Support (BLS) certification
